Job Description

VHC Health is seeking a qualified and compassionate Licensed Clinical Social Worker (LCSW) to fill an upcoming per diem shift on Wednesday, September 30, 2026.

In this role, you will provide critical psychosocial support, crisis intervention, and discharge planning services within our facility. We are looking for an independent professional who can seamlessly integrate into our care team and deliver high-quality, patient-centered care.

Responsibilities

  • Conduct comprehensive psychosocial assessments to determine patient and family needs.
  • Provide crisis intervention, emotional support, and short-term counseling to patients and their support systems.
  • Collaborate closely with multi-disciplinary healthcare teams (physicians, nurses, case managers) to coordinate effective discharge planning.
  • Facilitate referrals to community resources, home health agencies, and extended care facilities.
  • Maintain accurate, timely, and compliant electronic medical record (EMR) documentation according to hospital policies.

Qualifications & Requirements

  • License: Active, unrestricted license as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.
  • Education: Master’s Degree in Social Work (MSW) from an accredited program.
  • Experience: Minimum of 1–2 years of clinical social work experience, preferably in an acute care hospital or medical setting.
  • Skills: Excellent communication, conflict resolution, and case management skills. Ability to work efficiently under pressure in a fast-paced environment.

Schedule & Schedule Highlights

  • Day shift: 7:00 AM to 7:30 PM (12.5 hours total, includes a 30-minute unpaid break)
  • Single-day shift coverage with potential for future per diem opportunities.
