Travel Physical Therapist (Home Health)

Location: Renton / South King County, Washington

Start Date: September 7, 2026

Contract Length: 13 Weeks

Openings: 1

Schedule: Monday-Friday, 8:00 AM - 4:30 PM

Guaranteed Hours: 36 hours per week (40 scheduled)

Position Overview

Seeking an experienced Home Health Physical Therapist to provide patient care throughout South King County. This role focuses on evaluating, treating, and managing patients in the home setting while collaborating with physicians, nursing staff, and caregivers to help patients achieve optimal functional outcomes.

Key Responsibilities

• Complete comprehensive patient evaluations and develop individualized care plans
• Assess functional status and document findings in accordance with CMS guidelines
• Provide skilled physical therapy interventions in the home setting
• Monitor patient progress and communicate status changes to physicians and care teams
• Educate patients and caregivers on treatment plans, mobility, safety, and home exercise programs
• Complete all required documentation and OASIS assessments
• Supervise Physical Therapist Assistants (PTAs) as required
• Participate in interdisciplinary care conferences
• Prepare discharge summaries and progress reports
• Travel throughout assigned service areas and nearby agency territories when needed

Schedule & Coverage

• Monday-Friday
• 8:00 AM - 4:30 PM
• No routine on-call requirement listed
• Schedule flexibility required to meet patient needs
• May participate in weekend rotations and selected holiday coverage as needed
• Mileage reimbursement provided between patient visits

Requirements:

Experience

 Minimum 6 months of Home Health experience required Experience with:

• HCHB (Homecare Homebase)
• OASIS Documentation

Licensure & Certifications

• Active Washington Physical Therapist License OR Compact License with Washington privilege to practice
• Current BLS Certification
• Current Driver's License
• Proof of Vehicle Insurance
• Reliable Transportation

Pay Package:

Travel Rate

• Pay Package: $3,040/week
• Taxable Rate: $36/hr
• Housing Stipend: $1,316/week
• Meals & Incidentals: $644/week

Local Rate

• Pay Package: $2,680/week
• Hourly Rate: $75/hr
