Description
Description: A registered professional who provides physical therapy to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with interdisciplinary team.Essential Functions:1. Performs clinical assessment to identify patient needs and appropriateness of patient. 2. Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Works in consultation with the physician and as part of an interdisciplinary team.3. Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments. 4. Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devises and appliances in a safe manner.
Requires hand-eye coordination and manual dexterity.8. Promotes company philosophy and administrative policies.9. Must read, write and speak fluent English.SkillsCurrent State License as a Physical Therapist - RequiredProof of current CPR certification - RequiredCurrent driver's license. Must have reliable transportation and insurance - Required1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - RequiredPrevious home care experience - PreferredEducationBachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required Enable Skills-Based Hiring
