Registered Nurse (RN) – Step-Down Unit

Position Summary

The Registered Nurse (RN) – Step-Down provides comprehensive nursing care to patients who require a higher level of monitoring and care than a general medical-surgical unit but do not require full ICU-level care.

The RN is responsible for continuous cardiac monitoring, assessment of complex medical conditions, medication management, and timely intervention for changes in patient status. The ideal candidate should be experienced in managing cardiac, respiratory, neurological, and vascular patients in a fast-paced step-down environment.

Required Qualifications

• Specialty: RN – Step-Down
• Recent Experience: Minimum 2 years of recent Step-Down experience
• Guaranteed Hours: 36 hours per week
• Weekends: Every Other Weekend (EOW)
• Holidays: Every Other Holiday (EOH)
• Blocked Scheduling: Not available
• Patient Ratio: 1:4–5
• Local Candidates: Accepted
  • Candidates living within 50 miles of the facility are considered local.
  • Local bill rate is $15.00 less than the travel rate.

Unit / Patient Population

The Step-Down Unit cares for patients with conditions including:

• Congestive Heart Failure (CHF)
• Status post heart catheterization
• Status post vascular procedures
• Respiratory failure requiring BiPAP, CPAP, or Vapotherm
• Cardiac dysrhythmias

Unit Procedures & Clinical Expectations

• Post-heart catheterization care
• TR band management
• Post-vascular procedure monitoring
• Frequent vital-sign monitoring
• Neurovascular assessments and checks

Required Certifications

• BLS
• ACLS
• NIHSS

Required Clinical Skills

Cardiac / Emergency

• Cardiac monitoring
• Dysrhythmia interpretation
• Dysrhythmia management
• 12-lead EKG/ECG

IV / Vascular

• IV initiation and maintenance
• PICC line management
• Blood product administration

Medication Management

• Emergency medication administration
• Procedural sedation administration and monitoring
• Cardiac drip titration

Neurological

• CVA / acute stroke management

Respiratory / Thoracic

• BiPAP / CPAP management
• Assist with chest tube insertion and removal

Professional Skills

• Infection prevention practices
• Isolation precautions and appropriate PPE use
• Phlebotomy

Ideal Candidate

The ideal candidate is an experienced Step-Down RN with strong cardiac monitoring and acute-care skills. The candidate should be comfortable managing patients with cardiac dysrhythmias, respiratory failure, acute stroke/CVA, and post-procedural needs while maintaining safe patient ratios and responding promptly to changes in patient condition.
