Location: Urbandale, Iowa 50322
Schedule: Mon - Fri 9 - 5
On-Call: Possible
Travel: Local travel required; reliable transportation and current auto insurance required

Position Summary
The Registered Nurse (RN) provides skilled nursing care to home health patients in their residences, ensuring quality patient outcomes through comprehensive assessment, care planning, treatment, education, and coordination with the interdisciplinary healthcare team.
Key Responsibilities
- Perform comprehensive assessments and reassessments of home health patients, identifying physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate individualized Plans of Care (POC) and provide skilled nursing interventions.
- Manage medications, wounds, IV care, catheters, drains, pain management, phlebotomy, and other medical/surgical nursing needs.
- Educate patients and caregivers on medications, disease management, safety, infection control, nutrition, and self-care.
- Maintain accurate and timely clinical documentation in Homecare Homebase (HCHB) and complete required OASIS assessments.
- Coordinate care with physicians, patients/families, referral sources, and interdisciplinary team members while supervising HHA/LVN staff as applicable.
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license in Iowa.
- 2+ years of home health experience.
- 1+ year of Homecare Homebase (HCHB) experience.
- 1+ year of OASIS experience.
- 1–2 years of RN experience in a clinical care setting.
- Current CPR certification.
- Current driver's license, reliable transportation, and auto insurance. :
- Strong clinical assessment, communication, documentation, and organizational skills.
- Ability to work independently and travel locally.
Preferred Qualifications
- CHPN (Certified Hospice and Palliative Nurse) certification.
- Bachelor of Science in Nursing (BSN).
Education
- Required: Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
- Preferred: Bachelor of Science in Nursing (BSN).
