RN Care Manager – Inpatient Case Management | Direct Hire

Employment Type: Permanent / Direct Hire (Full-Time)

Location: White Plains, NY (Westchester County)

Shift / Schedule: Day Shift | 9:00 AM – 5:00 PM | Monday – Friday (Alternating Weekends & Holidays) | 37.5 Hours/Week

Hourly Rate / Annual Salary: $63.67 – $81.07 / hour ($124,156 – $158,086 annualized, commensurate with experience)

Position Overview

In this direct-hire hospital role, you will lead comprehensive admission assessments, utilization review (UR), transitional discharge planning, and interdisciplinary care coordination to optimize length of stay (LOS) and prevent readmissions across inpatient and emergency services.

Key Responsibilities

• Conduct comprehensive admission assessments, identify high-risk clinical indicators, and facilitate individualized discharge planning.
• Perform utilization review (UR) activities, including commercial payer reviews, admission clinical reviews, and length-of-stay (LOS) management using InterQual / Milliman criteria.
• Collaborate with physicians, bedside RNs, social workers, and post-acute providers to ensure seamless, safe transitions of care.
• Identify high-utilizer root causes and arrange timely post-discharge services, durable medical equipment (DME), and home care.

Qualifications & Requirements

• Licensure: Active, unencumbered New York State Registered Nurse (RN) license required.
• Education: Bachelor of Science in Nursing (BSN) or MSN required.
• Clinical Experience: Minimum 3+ years of acute care clinical nursing experience in Emergency Department (ED), Critical Care/ICU, or Med/Surg.
• Case Management Experience: 1–3+ years of dedicated acute care Case Management / Utilization Review experience required.
• Criteria Knowledge: Hands-on experience utilizing Milliman (MCG) or InterQual clinical guidelines required.
• Certifications: PRI Certification preferred (must be obtained within 3 months of hire); CCM (Certified Case Manager) or Screen certification strongly preferred.
