Description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.

 

Work Location: Midland, Michigan, 48642

 

Coverage Area: 60 Mile Radius

Pay Rate: $70.00/hour W-2

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Hours: 40 hours/week

Productivity: 30 points

On-Call/Weekend: None

Mileage: Reimbursed between patient visits

Position Summary:

  • We are seeking a Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes throughout the assigned coverage area.
  • The PT will conduct evaluations, provide patient education, develop plans of care, and oversee the care provided by PTAs.

Key Responsibilities:

  • Perform home health physical therapy evaluations and assess patients' functional needs.
  • Provide skilled PT services and education to patients and their families.
  • Develop and oversee individualized physical therapy plans of care.
  • Provide clinical direction and oversight for Physical Therapist Assistants (PTAs).
  • Monitor patient progress and modify treatment plans as appropriate.
  • Collaborate with physicians and the interdisciplinary home health team.
  • Complete accurate and timely clinical documentation.
  • Ensure care is provided in accordance with applicable home health standards and regulations.

Required Qualifications:

  • Current Michigan Physical Therapist (PT) license required.
  • Strong clinical assessment and communication skills.
  • Ability to provide care independently in patients' homes.
  • Preferred Qualifications
  • Previous home health PT experience.
  • Experience with Home Care Home Base (HCHB).
  • Experience overseeing PTA plans of care.
  • Dress Code: Any / No specific color requirement.
