Description
Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.
Location: Bourne, MA 02532
Weekly Gross Pay: Up to $3,000.00
Weekly Guarantee: 32 hours/week
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Patient Care Setting: Home Health
Job Type: Full-time
Productivity: 30 points
On-Call/Weekend: None
Mileage: Reimbursed between patient visits
Coverage Area: Team A territory, covering Cape Cod from the Canal to East Harwich.
Position Overview:
- We are seeking a dedicated Home Health Physical Therapist to provide skilled physical therapy services to patients in their homes.
- The PT will be responsible for evaluations, Start of Care visits, ongoing therapy visits, and collaboration with the care team to support positive patient outcomes.
Key Responsibilities
- Perform Physical Therapy Starts of Care (SOC) and comprehensive patient evaluations.
- Provide skilled PT visits and develop individualized treatment plans.
- Assess patients' mobility, strength, balance, functional limitations, and home safety needs.
- Provide appropriate therapeutic interventions to improve mobility and independence.
- Coordinate patient care with physicians, nurses, therapists, patients, and families.
- Hand off patients to and provide appropriate supervision of Physical Therapist Assistants (PTAs).
- Document patient care accurately and timely according to home health standards.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Promote patient safety and functional independence in the home environment.
Required Qualifications
- Current and unrestricted Massachusetts Physical Therapist (PT) license.
- Prior Home Health Physical Therapy experience required.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently while effectively collaborating with an interdisciplinary team.
- Reliable transportation and ability to travel throughout the assigned coverage area.
Preferred Qualifications
- Familiarity with HCHB (Homecare Homebase).
- Experience performing Home Health Starts of Care and supervising PTAs.
Compensation & Benefits:
- Up to $68.00/hour W-2
- Guaranteed 32 hours per week
- Mileage reimbursement between patient visits
- Monday–Friday schedule
- No weekends or on-call requirement
