Coverage Areas: Shelby/Fayette/Tipton
Shift schedule (days, start & end times): 8-4:30 M-F
Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)? Y
What are the top responsibilities for this position? (Please be as detailed as possible): case mamangement, admissions, IDT prep
Must have qualifications/experience: Hospice experience and HCHB a must
