Physical Therapist (PT) – Outpatient

Facility: VibrantCare Rehabilitation – Santa Cruz
Position Type: Full-Time | 13-Week Contract
Schedule: 40 Hours/Week
Shift: Primarily Monday–Friday, 8:00 AM–5:00 PM

Job Summary

We are seeking a skilled and compassionate Physical Therapist (PT) for a 13-week outpatient contract at VibrantCare Rehabilitation in Santa Cruz. The PT will provide high-quality, patient-centered rehabilitation services by evaluating patients, developing individualized treatment plans, and helping patients improve mobility, strength, function, and overall quality of life.

Responsibilities

• Evaluate patients and assess physical abilities, mobility, strength, and functional limitations.
• Develop and implement individualized treatment plans based on patient needs and clinical goals.
• Provide therapeutic exercises, manual therapy, and other appropriate physical therapy interventions.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and families on exercises, treatment plans, and strategies for continued improvement.
• Maintain accurate and timely clinical documentation.

• Collaborate with physicians, therapists, and other healthcare professionals to support positive patient outcomes.

   

Requirements

• Current Physical Therapist (PT) license in the applicable state.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Strong organizational and time-management skills.
• Previous outpatient physical therapy experience preferred.
