Description
Pride Health is actively hiring a Licensed Practical Nurse (LPN) to join our client's Home Health Team.
Location: Spokane, WA 99201
Pay Rate: Up to $37.00/hour on W-2
Setting: Home Health
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Productivity: 30 points
Coverage Area: Spokane
On-Call/Weekend: None
Mileage: Reimbursed between patient visits
Dress Code: Scrubs
Position Summary
We are seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout the Spokane area. The LPN will work closely with patients, families, and the clinical care team to support quality patient outcomes and continuity of care.
Key Responsibilities
- Provide skilled nursing care to patients in a home health setting according to the established plan of care.
- Perform patient assessments, monitor health status, and report changes to the supervising RN or clinical team.
- Administer medications and treatments as ordered and provide appropriate patient education.
- Document patient care, progress, interventions, and clinical observations accurately and timely.
- Communicate effectively with patients, caregivers, physicians, RNs, and other members of the interdisciplinary care team.
- Maintain compliance with applicable clinical, safety, and home health standards.
Required Qualifications
- Current Washington State LPN license.
- Home Health experience preferred.
- Strong clinical, communication, organizational, and documentation skills.
- Ability to travel between patient homes within the Spokane coverage area.
- Reliable transportation and ability to meet productivity expectations.
Nice-to-Have Skills
- Previous home health or community-based nursing experience.
- Familiarity with electronic medical records (EMR).
- Strong time-management and independent working skills.
- Experience working with patients with multiple or chronic conditions.
Certification Required: LPN License
