What You'll Do

• Provide skilled physical therapy services to residents in a SNF setting.
• Perform evaluations and develop individualized treatment plans.
• Help patients improve strength, balance, mobility, and functional independence.
• Assist patients with recovery following illness, injury, surgery, or hospitalization.
• Collaborate with nursing, physicians, and the interdisciplinary rehab team.
• Complete accurate and timely clinical documentation.

Requirements

• Active North Carolina Physical Therapist license or eligibility to obtain.
• SNF/LTC experience preferred.
• Current BLS certification.
• Strong communication and patient care skills.
