Description

Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.


Location: Andalusia, AL 36420
Coverage Area: Covington, Conecuh & Butler Counties
Pay Rate: Up to $58.00/hour on W-2
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekly Guarantee: 40 hours
Productivity: 30 points
Mileage: Reimbursed between patient visits
On-Call/Weekend: Yes
Dress Code: Royal Blue Scrubs

Job Summary:

  • We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Covington, Conecuh, and Butler counties.
  • The RN will perform comprehensive assessments, develop individualized care plans, provide skilled clinical interventions, and coordinate care with physicians, patients, families, and the interdisciplinary team.

Key Responsibilities

  • Provide skilled nursing care to patients in a home health setting, including comprehensive assessments, medication management, disease management, and patient education.
  • Complete accurate and timely clinical documentation and OASIS assessments using Homecare Homebase (HCHB).
  • Develop, implement, and update individualized plans of care based on patient needs and physician orders.
  • Perform skilled nursing procedures including wound care, wound VAC management, IV site care and maintenance, and venipuncture.
  • Monitor patient progress, identify changes in condition, and communicate appropriately with physicians and the interdisciplinary care team.
  • Educate patients and caregivers regarding medications, treatment plans, disease management, safety, and appropriate home care practices.
  • Maintain required productivity of 30 points while providing high-quality, patient-centered care.
  • Participate in required on-call and weekend rotations.

Required Qualifications

  • Active Registered Nurse (RN) license in the State of Alabama.
  • Current BLS/CPR certification.
  • Previous Home Health nursing experience.
  • Experience documenting in Homecare Homebase (HCHB).
  • Ability to travel throughout the assigned coverage area.
  • Valid driver's license, reliable transportation, and ability to meet field-based job requirements.
