Job Summary

The Critical Access Hospital (CAH) Med-Surg RN provides comprehensive, direct nursing care across a diverse patient population. Due to the unique environment of a Critical Access facility, this float/multi-specialty role requires clinical adaptability to care for adult, pediatric, orthopedic, and long-term care (LTC) patients. The RN is responsible for assessment, care planning, medication administration, post-operative care, and continuous patient monitoring, ensuring safe care delivery in low-volume, high-variance rural healthcare settings.

Key Responsibilities

1. General & Med-Surg Nursing (Adult & Peds)

• Perform physical assessments and develop patient-centered care plans for pediatric, adult, and geriatric admissions.
• Administer medications (IV, PO, IM, SubQ), blood products, and complex wound treatments following facility safety protocols.
• Manage pediatric admissions, adapting care plans to meet age-specific growth, development, and vital sign criteria.
• Continuously evaluate patient condition, rapidly identifying status changes and responding to emergency/resuscitation situations.

2. Orthopedic & Post-Surgical Care

• Manage pre- and post-op care for surgical/ortho patients (e.g., total joint replacements, fracture fixations, wound care management).
• Perform neurovascular assessments (CPM machines, compartment syndrome monitoring, traction care).
• Implement pain management strategies and early mobility/rehabilitation protocols in collaboration with Physical Therapy.

3. Long-Term Care (LTC) & Swing Bed Operations

• Manage chronic conditions, rehabilitation goals, and daily living support for LTC/Swing Bed residents.
• Oversee skin integrity, pressure ulcer prevention, fall risk mitigation, and restorative care plans.
• Facilitate discharge planning, interdisciplinary care conferences, and family updates.

4. Critical Access Facility Operations

• Float flexibly across Med-Surg, Peds, Ortho, and LTC units based on census and acuity needs.
• Direct and delegate tasks effectively to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
• Maintain compliance with regulatory frameworks (CMS Critical Access guidelines, Joint Commission, state regulations).

Education

Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.

Licensure

Active, unencumbered RN License in state of practice (or Compact/NLC license).

Certifications

• BLS (Basic Life Support) — Required

• ACLS (Advanced Cardiovascular Life Support) — Required

• PALS (Pediatric Advanced Life Support) — Required within 6 months of hire

• NIHSS — Preferred

Experience

1–2+ years of acute care Med-Surg, Ortho, Peds, or LTC experience. Critical Access or float experience highly valued.

Skills

EMR proficiency, IV insertion, telemetry interpretation, age-specific competency (Peds to Geriatrics), strong multi-tasking skills.

Physical Requirements & Working Conditions

• Ability to stand, walk, bend, and lift up to 50 lbs unassisted.
• Exposure to biological hazards, infectious diseases, and medical equipment.
• Flexibility to work weekends, holidays, and on-call rotations as required by hospital operations.
