Licensed Practical Nurse (LPN)

Location: Laconia, NH
Assignment Length: 12 Weeks
Shift: Nights | 6:45 PM – 7:15 AM
Guaranteed Hours: 36 Hours/Week

Job Summary

We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide quality nursing care under the direction of a Registered Nurse (RN). The LPN will provide direct patient care, administer medications and treatments, monitor patient conditions, and collaborate with the nursing team to support positive patient outcomes.

Responsibilities

• Provide direct nursing care to patients in accordance with the care plan.
• Monitor, assess, document, and report changes in patient condition to the RN.
• Administer medications and treatments as ordered.
• Assist with patient care planning, discharge preparation, and patient/family education.
• Supervise and delegate appropriate tasks to unlicensed nursing staff.
• Perform medication/drug counts as required.
• Assist physicians during rounds and examinations.
• Maintain accurate documentation using PointClickCare (PCC).
• Communicate effectively with patients, families, and interdisciplinary team members.
• Follow all facility policies, safety standards, HIPAA, and applicable regulations.
• Participate in required training, quality improvement activities, and staff orientation.

Requirements

• Graduate of an approved Practical/Vocational Nursing program.
• Current LPN license in the state of New Hampshire.
• BLS/CPR certification required.
• Experience in geriatric, skilled nursing, or rehabilitation care preferred.
• Knowledge of medications, nursing procedures, and emergency response.
• Ability to communicate effectively in English.
• Basic computer skills.
