We are seeking a Travel Occupational Therapist for a 13-week Skilled Nursing Facility contract in Holmes, New York. Responsibilities

• Evaluate patients’ functional abilities and develop individualized treatment plans.
• Provide skilled occupational therapy services in accordance with physician orders and facility protocols.
• Support patients with ADLs, safety, mobility, upper extremity function, cognition, and functional independence.
• Document patient progress and maintain accurate treatment records.
• Collaborate with nursing staff, PT, SLP, and the interdisciplinary rehab team.
• Educate patients and caregivers on safety strategies, adaptive equipment, and home program recommendations.
