๐ Long-Term Contract Opportunity
Are you a compassionate and motivated Registered Nurse who enjoys health education, preventative care, and making a positive impact in the community? Join a dynamic Wellness Department where you'll help members improve their overall health through screenings, education, vaccinations, and outreach initiatives.
๐ Responsibilities:
โ Perform health screenings and monitor acute and chronic conditions
๐ฉบ Conduct blood pressure, glucose, A1C, lipid, BMI, and other wellness assessments
๐ Administer seasonal vaccinations and other immunizations
๐ Participate in Health Fairs and community outreach events throughout NYC and the surrounding boroughs
๐ Educate members in person, virtually, and over the phone on disease prevention, chronic disease management, and healthy lifestyle choices
๐ Accurately document clinical data for reporting and statistical analysis
๐ค Counsel members on available 1199SEIU health benefits and connect them with appropriate internal departments and community resources
๐ Communicate with physicians and specialists regarding member care and follow-up
๐ Complete additional wellness initiatives and special projects as assigned
โ Qualifications:
โ๏ธ Active New York State Registered Nurse (RN) License required
๐ Bachelor's Degree in Nursing preferred
๐ฅ Minimum 2 years of RN experience in Ambulatory Care, Community Health, or Medical-Surgical Nursing
๐ง Strong clinical assessment, patient education, motivational interviewing, critical thinking, and independent decision-making skills
๐ป Comfortable using Microsoft Outlook, Word, Excel, and electronic documentation systems
๐ฃ๏ธ Excellent communication and interpersonal skills
๐ Bilingual Spanish is a plus!
๐ Valid NYS Driver's License preferred with the ability to travel throughout the New York Metropolitan area
๐ผ Ability to carry a laptop and work occasional evenings or weekends as needed
