Position: Clinical Coordinator II – RN/BSN
Facility: Columbia University Medical Center
Department: Clinical Operations
Location: New York, NY
Position Summary
Reporting to the Director of Clinical Operations, the Clinical Coordinator II – RN assists physicians with patient care, health education, EMR chart management, care coordination, and clinical protocols.
Responsibilities
- Assess and collect patient health information and document in the EMR.
- Coordinate patient care across inpatient, outpatient, home, skilled nursing, and school settings.
- Assist with medication management and prior authorizations.
- Administer immunizations and medications.
- Provide telephone, MyChart, and in-person triage and patient education.
- Assist with urgent and emergent patient situations.
- Coordinate multidisciplinary services and diagnostic testing.
- Review and act on lab and imaging results.
- Manage patient communications, immunizations, and health maintenance updates.
- Ensure infection control compliance and support patient/family satisfaction.
- May float to other clinics based on operational needs.
- May precept new clinical staff, including MAs, LPNs, and junior nurses.
- Perform other duties as assigned.
Requirements
- BSN / Bachelor's Degree in Nursing or equivalent education and experience.
- 2–4 years of related experience.
- Current New York State RN License.
- BLS Certification required.
- Strong patient care, communication, triage, EMR, and care coordination skills.
