Position: Clinical Coordinator II – RN/BSN

Position Summary

Reporting to the Director of Clinical Operations, the Clinical Coordinator II – RN assists physicians with patient care, health education, EMR chart management, care coordination, and clinical protocols.

Responsibilities

• Assess and collect patient health information and document in the EMR.
• Coordinate patient care across inpatient, outpatient, home, skilled nursing, and school settings.
• Assist with medication management and prior authorizations.
• Administer immunizations and medications.
• Provide telephone, MyChart, and in-person triage and patient education.
• Assist with urgent and emergent patient situations.
• Coordinate multidisciplinary services and diagnostic testing.
• Review and act on lab and imaging results.
• Manage patient communications, immunizations, and health maintenance updates.
• Ensure infection control compliance and support patient/family satisfaction.
• May float to other clinics based on operational needs.
• May precept new clinical staff, including MAs, LPNs, and junior nurses.
• Perform other duties as assigned.

Requirements

• BSN / Bachelor's Degree in Nursing or equivalent education and experience.
• 2–4 years of related experience.
• Current New York State RN License.
• BLS Certification required.
• Strong patient care, communication, triage, EMR, and care coordination skills.
