LPN – Post-Acute Skilled Nursing/Rehab | Simplified Job Posting
Facility: Helen Porter Nursing & Rehab Center
Beds: 98
Unit: Post-Acute Care / Skilled Nursing Rehab
Shift: 12-hour Days
Contract: 35 weeks
Guaranteed Hours: Not specified
EMR: Epic
Patient Population: Adults 60+
Patient Ratio: 1:14
Must-Have Requirements
- LPN license – Vermont (VT) required at submission
- BLS required
- Minimum 1 year clinical/LPN experience
- Prior travel experience required
- Epic experience within the past 2 years – mandatory
- Reliable transportation required
- Must be able to work all holidays
- Must work at least every other weekend or according to unit needs
Preferred
- Dementia-capable care certification
- IV certification
- Charge experience preferred
Duties
- Provide nursing care to skilled nursing/rehab patients
- Administer medications and treatments
- Monitor patient conditions and report changes
- Perform phlebotomy and EKGs
- Start IVs
- Manage IV pumps
- Perform/assist with patient transports
- Administer nebulizer-to-inhaler treatments
- Document patient care in Epic
- Communicate with CNAs/LNAs, providers, and other healthcare staff
- Follow post-acute care and safety protocols

Staffing
- Nurse ratio: 1:14
- 3 staff per shift
- 1 unit clerk on day shift
Scheduling
- No block schedules
- No guaranteed days off unless written into the contract
- Weekend requirements apply
- Must be available for all holidays
- Orientations occur every other Monday
Important Submission Requirements
- VT license must be active at submission
- Epic experience from the last 2 years
- All employment gaps over 90 days must be explained
- Complete work history required, including facility type, bed count, EMR, etc.
- Background checks required:
- 7-year nationwide/county/FACIS 3/sex offender check within 18 months of start
- FACIS 3 + VCIC within 30 days of start
