Registered Nurse – Med Surg Step Down (RN Step Down)

Shift Matrix: Day / Night Rotation (D/N) | 3x12-Hour Shifts

  • Shift Times: Rotates between Day Shift (07:00 – 19:30) and Night Shift (19:15 – 07:15).
  • Schedule Quality: 36 hours per week. Weekend and holiday rotations required.
  • On-Call Requirement: No call.

🚫 Absolute Minimum Qualifications & Departmental Hardstops:

  • Licensure Mandate: Active, unencumbered Massachusetts State RN License held in hand at submission.
  • Experience Baseline: Minimum 2+ years of continuous Med Surg Step Down / Progressive Care Unit (PCU) RN experience in an acute care setting.
  • Core Certifications Required:
    • Active AHA BLS
    • Active AHA ACLS
  • Clinical Skill Mandate: Demonstrated telemetry strip interpretation skills, vasoactive drip titrations, high-acuity respiratory monitoring, and post-procedural progressive care management.
  • Shift Flexibility: Candidate must explicitly commit to full Day/Night (7a/7p) rotation and holiday/weekend schedule requirements.

Job Summary & Clinical Scope

Newton-Wellesley Hospital (MGB) in Newton, MA is seeking an experienced, flexible Med Surg Step Down Registered Nurse (RN Step Down) for a 13-week D/N rotation assignment on unit 4 USEN (Department NW6026 / SO00003634). Caring for intermediate-acuity patients who require continuous telemetry and frequent clinical monitoring, the RN will direct progressive care interventions within a community teaching hospital network.

  • Progressive Care & Step Down Monitoring: Manage intermediate-acuity medical and surgical patients requiring continuous cardiac telemetry, pulse oximetry, arterial line monitoring, and frequent neurological/hemodynamic assessments.
  • Pharmacologic & Drip Management: Administer continuous intravenous medications, manage titratable vasoactive infusions per protocol, and monitor for therapeutic efficacy and adverse drug reactions.
  • Respiratory & Post-Surgical Care: Support complex respiratory patients (non-invasive ventilation/BIPAP, high-flow nasal cannula) and manage post-operative surgical recoveries transitioning out of the ICU.
  • Interdisciplinary Coordination & Charting: Execute precise clinical documentation and collaborate daily with attending hospitalists, surgical teams, case managers, and multi-disciplinary therapies.
