Job Summary

We are seeking experienced Registered Nurses for a Nurse Assessor position serving patients in the Bronx/Parkchester area. This role involves conducting comprehensive in-home assessments, reviewing patient health needs, and supporting individualized long-term care plans.

Responsibilities

• Conduct comprehensive in-home assessments of patients' health status, strengths, needs, and preferences.
• Review medical records, referrals, and supporting documentation.
• Complete and electronically submit patient assessments.
• Identify care needs and support individualized long-term care service plans.
• Discuss healthcare options, available services, and community resources with patients.
• Coordinate with interdisciplinary care teams and healthcare providers.
• Document assessment findings, concerns, and case notes accurately.
• Support continuity of care and help reduce gaps or duplication in services.
• Conduct both in-field and telehealth assessments.

Requirements

• Current New York State RN license required.
• Minimum 2 years of experience working in a home setting within the last 15 years.
• Recent hands-on experience conducting comprehensive patient assessments required.
• Home Health, CHHA, LHCSA, MLTC, hospice, assisted living, or other home-based care experience accepted.
• UAS-NY assessment experience strongly preferred.
• Bilingual Spanish-English proficiency strongly preferred.
• BLS required before the start date.
• Must be comfortable working independently in a fast-paced, technology-driven environment.
• Must be available for hybrid telehealth and in-field assessments.
• Candidates cannot perform UAS assessment assignments elsewhere and must complete a conflict-of-interest form before starting.
