Hospice Social Worker – Travel Assignment
Department: Evergreen Hospice – Samaritan Health Services
Shift: Days | 8:00 AM–4:30 PM
Weekends: None
Guaranteed Hours: 24 hours/week
Contract Length: 91 weeks
Position Overview
Evergreen Hospice provides supportive care to patients and families during the final stages of illness. Services are delivered in home-based settings, including:
- Patient residences
- Skilled nursing facilities
- Assisted living facilities
- Adult foster homes
The hospice team provides comprehensive support focused on patient comfort, symptom management, and family needs.
Services Provided
The hospice program includes:
- Intermittent skilled nursing visits
- 24-hour on-call nursing support
- Pain and symptom management
- Medical social services
- Counseling
- Spiritual support
- Hospice aide services
- Trained hospice volunteers
- Bereavement support
- Medications and medical equipment as appropriate
Required Qualifications
- Oregon LMSW (Master’s-level Social Worker) or CSWA (Clinical Social Work Associate) license required
- Healthcare Provider Level BLS required
- Valid driver's license required
- Personal automobile required for work
- Auto liability/property damage insurance as required by law
- Personal vehicle must be maintained in good working condition
Preferred Qualifications
- Oregon LCSW preferred
- Previous hospice experience preferred
- Social work experience in a hospital or medical setting preferred
