Position Summary

We are seeking an experienced Registered Nurse (RN) Case Manager to support Purchased Referred Care (PRC) Case Management services at the Chenega Fort Peck Service Unit. The RN Case Manager will coordinate patient care, referrals, and healthcare services while ensuring appropriate documentation, communication, and continuity of care.

The ideal candidate will have strong clinical knowledge, case management experience, and the ability to coordinate effectively with patients, healthcare providers, and internal teams.

Key Responsibilities

• Provide professional RN case management and care coordination services.
• Coordinate patient referrals and healthcare services through the Purchased Referred Care (PRC) program.
• Review patient needs and assist with appropriate healthcare resource coordination.
• Coordinate with physicians, specialists, hospitals, clinics, and other healthcare providers.
• Monitor patient care plans, referrals, appointments, and follow-up services.
• Assist with determining appropriate care and ensuring continuity of care.
• Review and maintain accurate patient records and case documentation.
• Communicate effectively with patients, families, providers, and healthcare teams.
• Support timely processing and coordination of healthcare referrals.
• Maintain compliance with applicable clinical, organizational, and program requirements.
• Protect patient confidentiality and handle sensitive healthcare information appropriately.
• Perform other case management and nursing duties as assigned.

Required Qualifications

• Current Registered Nurse (RN) license.
• Previous experience in RN case management, care coordination, utilization management/review, or a related nursing function.
• Strong clinical assessment and communication skills.
• Experience coordinating care across multiple healthcare providers.
• Strong documentation and organizational skills.

• Ability to work independently and manage multiple cases effectively.

   

Preferred Qualifications

• Experience with Purchased Referred Care (PRC).
• Previous experience with Indian Health Service (IHS).
• Experience working in a federal, tribal, rural, or community healthcare setting.
• Experience with referral management and utilization review.
• Knowledge of healthcare eligibility, authorizations, and medical necessity processes.
