Clinical Coordinator II – RN/BSN
Facility: Columbia University Medical Center
Location: New York, NY
 

Position Details

• Clinical coordination and patient care support role reporting to the Director of Clinical Operations
• Assists physicians with patient care, health education, EMR management, and care coordination

Key Responsibilities

• Assess patient health status and document in the EMR
• Coordinate patient care plans, medications, renewals, and prior authorizations
• Perform nursing interventions, immunizations, and medications
• Handle telephone, MyChart, and in-person triage
• Coordinate diagnostic testing and multidisciplinary services
• Review and follow up on lab and imaging results
• Provide patient/family education and support urgent/emergent situations
• Maintain infection control and patient satisfaction standards
• May float between clinics and precept junior clinical staff

Requirements

• 2–4 years of related experience
• Current New York State RN License
• BLS certification
• Must be able to work in a clinical environment with exposure to blood/body fluids
• Travel between clinical sites may be required
