Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Location: Jacksonville, FL, 32216
Coverage Areas: Nassau & Duval County, FL
Pay Rate: Up to $58.00/hour
Setting: Home Health
Schedule: Monday-Friday, 8:30 AM - 5:00 PM
Weekly Hours Guaranteed: 40 Hours
Productivity Expectation: 30 Points
On-Call/Weekend Requirement: Every Other Weekend
Mileage Reimbursement: Yes (between patient visits)
Dress Code: Any Solid Color Scrubs

Position Overview
- We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team serving patients throughout Nassau and Duval County.
- The ideal candidate will be responsible for delivering high-quality skilled nursing care, conducting comprehensive assessments, coordinating care plans, and ensuring compliance with home health regulations and documentation standards.
- This role requires strong clinical judgment, OASIS expertise, proficiency with HCHB/PointCare, and the ability to work independently while collaborating effectively with physicians and interdisciplinary care teams.

Required Qualifications
- Active Registered Nurse (RN) license.
- Current CPR Certification.
- Valid Driver's License.
- Current vehicle insurance and access to a dependable vehicle.
- Proficiency with HCHB/PointCare.
- Strong OASIS assessment and documentation experience.
- Home Health nursing experience preferred.
- Ability to work independently in a fast-paced field environment.

- Conduct accurate patient assessments, OASIS evaluations, and medication reconciliation while ensuring compliance with home health regulations and payer requirements.
- Develop and manage individualized plans of care in collaboration with physicians and interdisciplinary healthcare teams.
- Deliver high-quality skilled nursing services and oversee clinical care provided by LPN/LVNs through effective supervision and guidance.
- Maintain timely, accurate, and compliant clinical documentation using HCHB/PointCare and agency-approved systems.
- Coordinate patient care, physician communication, scheduling updates, and family education to achieve positive patient outcomes and service excellence.
