Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Location: Philipsburg, PA16866
Coverage Area: Clearfield and Centre County, PA
Weekly Gross Pay: Up to $2,516.00
Weekly Hours Guaranteed: 36 Hours
Shift Schedule: Monday-Friday, 8:00 AM - 4:30 PM
On-Call/Weekend Requirement: Yes
Mileage Reimbursement: Yes, reimbursed between patient visits (from first patient to last patient)
Patient Care Setting: Home Health
Position Overview:
- We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Clearfield and Centre County, PA.
- The ideal candidate will have strong home health experience and be proficient in OASIS assessments, care planning, patient education, wound care, and coordination of care.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and ongoing patient evaluations.
- Develop, implement, and update individualized patient care plans.
- Perform routine and follow-up home health visits.
- Provide skilled nursing interventions in accordance with physician orders and agency protocols.
- Administer IV therapies and perform venipuncture as needed.
- Manage and monitor wound care treatments, including wound VAC therapy.
- Educate patients and caregivers on disease processes, medications, treatment plans, and self-care management.
- Accurately document patient assessments, interventions, and outcomes in compliance with regulatory standards.
- Collaborate with physicians, therapists, case managers, and interdisciplinary team members to ensure quality patient care.
- Participate in on-call and weekend coverage as required.
Required Qualifications
- Active Registered Nurse (RN) license in Pennsylvania.
- Minimum of one year of recent Home Health nursing experience.
- Strong knowledge and experience with:
- OASIS assessments
- Home-Based Health Care (HBHC)
- IV therapy administration
- Venipuncture
- Wound care and Wound VAC management
- Patient and caregiver disease education
- Care plan development and coordination
- Valid driver's license and reliable transportation.
Preferred Qualifications:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Ability to work independently and manage a mobile patient caseload.
- Experience working across diverse patient populations within a home health setting.
