JOB DESCRIPTION Montefiore Mount Vernon/ Medical management department is seeking a RN case manager to provide a range of direct and indirect services for patient/families including, but not limited to, the identification of psychosocial problems and their resolutions. In addition, the RN case manager collaborates with the patient/family/team to effect safe and timely discharge/treatment plans.
