Schedule: Monday–Friday, 8:00 AM–5:00 PM

Job Summary

The Registered Nurse provides skilled nursing care to patients in their homes according to physician orders and agency policies. The RN coordinates and directs patient care, performs assessments and visits, and works closely with the care team to ensure quality patient outcomes.

Responsibilities

• Provide skilled nursing care during home visits.
• Perform Start of Care (SOC), wound care, follow-up visits, and other skilled nursing services.
• Coordinate patient care according to physician orders and the nursing process.
• Complete accurate and timely documentation.
• Supervise and evaluate Home Health Aides and LPN/LVNs as needed.
• Communicate with physicians, patients, families, and the healthcare team.
• Manage approximately 5–8 patients per day, depending on patient needs.
• Travel within the assigned territory, up to 75 miles from the home clinic.
• Maintain productivity expectations of approximately 28–30 units/points per week.

Requirements

• Minimum 1 year of recent Adult Home Health or Hospice experience.
• BLS/CPR required.
• Active RN license required; pending licenses are not accepted.
• Recent experience with HomeCare HomeBase (HCHB) required. Candidates who have not used HCHB within the last 6 months may be required to complete retraining.
• OASIS proficiency required.
• Reliable transportation, valid driver's license, and required auto liability insurance.
• Must be willing to travel for home visits.

• Mileage reimbursement provided at the IRS standard rate for travel between patient homes.
• Dress Code: Scrubs.
• Guaranteed Hours: No.
