Quick Facility Summary

Shift: Variable PM/NOC | 48 hrs/week guaranteed
Contract: 6 weeks, with potential extension
Facility: 72 beds | ~60–65 census | 42-bed assignment
Units: Rehab/LTC + Dementia
Staffing: 3 nurses on AM/PM; 2 nurses on NOC
CNA Ratio: ~10–12 residents per CNA
EMR: Paper charting; transitioning to EMAR
Work Week: Sunday–Saturday
Services: Short-term rehab/ortho, LTC, dementia; in-house PT/OT/ST
Location: Ashland, WI, near Lake Superior; hiking, fishing, biking, and local dining
Orientation: Arrive 10–15 minutes before shift
Compliance: WI RN/LPN license, WI BID, WI DOJ, COVID vaccine or exemption
Not Required: MMR or Varicella
