Position Overview

Seeking a Clinical Pharmacist to provide safe and effective medication management across inpatient and/or outpatient settings. The pharmacist will review medication orders, optimize pharmacotherapy, collaborate with healthcare providers, provide patient education, and support pharmacy operations.
Key Responsibilities

• Review and validate medication orders and prescriptions for appropriateness and safety.
• Dispense medications, supplies, and equipment accurately and efficiently.
• Monitor patient medication and nutritional therapies and intervene when needed.
• Collaborate with physicians and interdisciplinary healthcare teams.
• Provide medication counseling and education to patients and healthcare staff.
• Participate in medication reconciliation and transition-of-care activities as assigned.
• Respond to medication-related inquiries from physicians and staff.
• Support pharmacy technician, intern, resident, and new pharmacist training.
• Maintain accurate clinical documentation and required records.
• Follow federal/state regulations and hospital policies regarding medication use.
• Participate in emergency responses and clinical rounds when assigned.
