Position Title: Physical Therapist (PT) – Home Health
Location: Bourne, MA 02532
Pay Rate: Up to $62.00/hour on W2
Patient Care Setting: Home Health
Schedule: Monday to Friday, 8:00 AM - 4:30 PM
Weekly Hour Guarantee: 32 Hours
Job Status: Open Until Filled
Coverage Area: Team A Territory including Cape Cod from the Canal to East Harwich
On-Call Requirement: None
Mileage Reimbursement: Yes, between patient visits (from first patient to last patient)
Productivity Expectation: 30 points
Position Overview:
- We are seeking a dedicated Home Health Physical Therapist (PT) to provide high-quality rehabilitation services to patients in their homes throughout the Cape Cod region.
- The ideal candidate will be responsible for conducting Start of Care assessments, comprehensive evaluations, ongoing therapy visits, and collaborating with Physical Therapist Assistants (PTAs) to ensure exceptional patient outcomes.
- This role offers a consistent weekday schedule with no on-call or weekend requirements.
Key Responsibilities:
- Perform Physical Therapy Start of Care (SOC) visits and comprehensive patient evaluations.
- Develop individualized treatment plans based on patient needs, goals, and physician orders.
- Provide skilled physical therapy interventions during home health visits.
- Assess patient progress and modify treatment plans as appropriate.
- Educate patients and caregivers on treatment plans, mobility strategies, safety precautions, and home exercise programs.
- Coordinate care with physicians, nurses, case managers, and other healthcare professionals.
- Delegate appropriate treatments to Physical Therapist Assistants (PTAs) and provide clinical supervision as required.
- Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines.
- Ensure compliance with home health regulations and quality standards.
Required Qualifications:
- Current and active Massachusetts Physical Therapist (PT) License.
- Previous Home Health Physical Therapy experience.
- Strong clinical assessment and patient care skills.
- Ability to work independently and manage a territory-based caseload.
- Valid driver's license and reliable transportation.
Preferred Qualifications:
- Experience using Homecare Homebase (HCHB) software.
- Familiarity with home health documentation and OASIS assessments.
- Excellent communication, organization, and care coordination skills.
