Responsibilities

Complete Start of Care (SOC), evaluations, reassessments, and discharges
Develop and manage patient plans of care
Supervise PTA staff
Manage and schedule your own patient caseload
Provide home health therapy services throughout York and Eastern Adams County

Requirements

Active Physical Therapist license
Graduate of an accredited PT program
CPR (if required)
1+ year of Home Health experience highly preferred
Home Care Home Base (HCHB) experience highly preferred
OASIS experience preferred
