Travel Occupational Therapist (OT) – Skilled Nursing Facility
Falmouth, Massachusetts
We are seeking a Travel Occupational Therapist for a 13-week Skilled Nursing Facility contract in Falmouth, Massachusetts. This opportunity is a great fit for an OT who enjoys post-acute rehab, geriatric care, and helping patients improve independence, safety, ADLs, mobility, and overall functional ability.
This role offers the chance to evaluate patients, create individualized treatment plans, and provide hands-on therapy in a collaborative rehab environment.

Responsibilities
- Evaluate patients’ functional abilities and develop individualized treatment plans.
- Provide skilled occupational therapy services in accordance with physician orders and facility protocols.
- Support patients with ADLs, safety, mobility, upper extremity function, cognition, and functional independence.
- Document patient progress and maintain accurate treatment records.
- Collaborate with nursing staff, PT, SLP, and the interdisciplinary rehab team.
- Educate patients and caregivers on safety strategies, adaptive equipment, and home program recommendations.
