Location: Philadelphia & Montgomery County, PA
Facility: Penn Medicine at Home
Contract Length: 13 weeks
Start Date: October 5, 2026
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40 guaranteed hours per week

Job Description

Penn Medicine at Home is seeking an experienced Physical Therapist for a 13-week local contract assignment. The therapist will provide home-based physical therapy services to patients throughout Northeast Philadelphia and Montgomery County, primarily covering ZIP codes 19141 and 19120.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans
• Provide therapeutic exercises, mobility training, and functional rehabilitation
• Assist patients with improving strength, balance, safety, and independence
• Educate patients, families, and caregivers on treatment plans and home exercises
• Document evaluations, treatments, progress, and outcomes accurately
• Coordinate care with physicians, nurses, and other members of the home-health team
• Travel between patient homes within the assigned territory

Requirements

• Active Pennsylvania Physical Therapist license
• Graduate of an accredited Physical Therapy program
• Previous home-health experience strongly preferred
• Current BLS certification
• Reliable transportation and valid driver’s license
• Strong communication, documentation, and time-management skills
• Ability to work independently in a home-care environment
