Position Details:

• Contract Length: 6 Months
• Schedule: Sunday–Saturday; shifts may alternate based on unit staffing needs

Available Shifts:

Candidates must be available for shifts based on unit needs:

• 6:00 AM – 2:00 PM
• 2:00 PM – 10:00 PM
• 10:00 PM – 6:00 AM
• 6:00 AM – 6:00 PM
• 10:00 AM – 10:00 PM
• 6:00 PM – 6:00 AM

Important: Candidates must be available to start on the 1st of the month. New Employee Orientation (NEO/NNO) is required Monday–Friday, 8:00 AM–5:00 PM during the first month.

Position Overview:

The LVN will provide advanced vocational nursing care to individuals at Wichita Falls State Hospital who may have intellectual disabilities, complex medical conditions, mental health needs, and/or behavioral health issues, under the guidance of the Facility Medical Director.

Key Responsibilities:

• Provide nursing care and services to individuals residing at the facility.
• Administer medications, immunizations, and treatments as ordered.
• Monitor individuals for unusual symptoms, reactions, and changes in condition.
• Monitor vital signs, weight, diet, and feedings.
• Identify signs and symptoms of illness and use appropriate nursing judgment to provide care.
• Initiate acute care plans when clinically appropriate.
• Provide nursing care according to the Nursing Process and Texas Board of Nursing requirements.
• Complete electronic data entry, medical records, reports, and other required documentation accurately and on time.
• Follow facility policies, Standard Operating Procedures, HHSC requirements, and applicable Joint Commission standards.
• Maintain compliance with all facility and HHSC rules, policies, and directives.

Requirements:

• Current LVN license and registration to practice in Texas or a license from a state participating in the Nurse Licensure Compact.
• Minimum 2 years of LVN experience required.
• Must be at least 18 years of age.
• Must not be excluded from providing services to Medicare/Medicaid beneficiaries.
• Must successfully complete drug screening and criminal history checks, including fingerprint and registry clearances.
• Must meet HHSC immunization requirements.
• Must complete all required facility and HHSC training, including facility CPR.
• Must meet all additional requirements established by the facility.

Compliance & Start Date:

The contract start date will begin only after all required compliance, onboarding, and clearance requirements have been successfully completed.
