Description
RN CARE MANAGER CONTRACT (No stipends)
DURATION: 10/12 - end of year (potential extension)
LOCATION: Springfield, MA
SCHEDULE: M-F 8-5
PAY: $45-53/HR (BASED ON EXPERIENCE)
*MUST BE COMFORTABLE GOING INTO PATIENT HOMES*
Day-to-Day Responsibilities
This community-based Care Manager supports dual-eligible Medicare and Medicaid members with complex medical, behavioral, and social needs. The primary focus is conducting assessments to improve health outcomes and quality of life
What you would be doing:
Meeting members in their homes and communities (you will be completing 10 assessments/week minimum)
Performing assessments and identifying care needs
Building member-centered care plans
Coordinating services and resources
Supporting members through healthcare transitions
Connecting members to social support programs
Promoting wellness and preventive care
Improving health outcomes through ongoing engagement
Must-Haves
• Current Massachusetts license as one of the following:
○ Registered Nurse (RN)
• Associate's Degree in Nursing
• Either:
○ 3+ years of RN/Behavioral Health Clinician experience, or
○ 1 year of RN/Behavioral Health experience plus 2 years working with complex populations
• Valid driver's license, reliable transportation, and auto insurance
