Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.

Location: Wallingford, CT 06492
Coverage Area: Middletown, CT and surrounding towns
Pay Rate: Up to $65.00/hour
Patient Care Setting: Home Health
Schedule: Monday-Friday, approximately 8:30 AM - 4:30 PM
Hours: 40 Hours per Week (8 Hours/Day)
Productivity Expectation: 30 Points
On-Call Requirement: No
Weekend Requirement: No
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Business Casual or Scrubs

Position Overview:

• We are seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team serving patients throughout Middletown and the surrounding communities.
• The Physical Therapist will provide skilled rehabilitative care in patients' homes, helping individuals improve mobility, restore function, reduce pain, and achieve the highest possible level of independence.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled home care physical therapy services focused on improving strength, balance, mobility, endurance, safety, and functional independence.
• Conduct ongoing patient assessments and modify treatment plans as appropriate to achieve optimal outcomes.
• Educate patients, families, and caregivers on therapeutic exercises, fall prevention, mobility techniques, safety strategies, and home exercise programs.
• Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines.
• Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate patient care.
• Maintain productivity standards while delivering exceptional patient-centered care.
• Participate in care conferences and contribute to individualized patient care planning.

Required Qualifications:

• Active Physical Therapist (PT) License in the State of Connecticut.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication, organization, and time-management abilities.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently in a home health environment.
