Description
Shift timings:
Day 3x12-Hour (08:00 - 16:30)
Work experience:
Acute medical hospital inpatient Social Work experience performing both roles of Care Coordination.
Charting System:
EPIC
Education:
MSW
License:
LCSW is preferred. (Not mandate)
