Description
Responsibilities
- Provide social work services for patients receiving dialysis treatment in outpatient settings.
- Support patients with renal failure and dialysis-related needs.
- Manage social work needs across three KDU dialysis center locations.
- Provide services through in-person visits and/or telehealth, depending on location and scheduling.
- Float/travel to sister locations, including Turtle Lake and Garrison, as needed with notice.
- Coordinate and communicate with the departmental manager and director regarding patient and departmental needs.
- Maintain accurate documentation in EPIC.
- Complete required orientation and work with a preceptor during the first week, with preceptor support available throughout the assignment.
- Maintain professional business-casual attire for meetings.
Requirements
MSW or LCSW required.
ND Social Work licensure required at submission.
LCSW license required per job requirements.
BLS certification required.
Minimum 1 year of social work experience.
Previous travel experience not required.
Experience in an outpatient dialysis setting is relevant.
Must be willing and able to travel/float to Turtle Lake and Garrison when needed.
Must meet the 50-mile radius rule.
Cannot currently be full-time, part-time, or PRN with any CommonSpirit/CHI/Dignity facility.
RTO must be submitted upfront: up to 7 days for a 13-week contract; additional RTO will not be approved.
Must be available for 40 guaranteed hours per week, 8:00 AM–5:00 PM schedule.
No weekend or holiday requirement.
BSN is not required.
