Looking for your next Social Work travel opportunity? Join a Hospital Care Management team in Billings, Montana, where you'll have the opportunity to make a meaningful impact by supporting patients and families throughout their healthcare journey.

This role focuses on care coordination, discharge planning, psychosocial assessment, resource connection, and multidisciplinary collaboration. You'll work closely with patients, families, physicians, nurses, and other healthcare professionals to help ensure appropriate care and support during hospitalization and transition to the next level of care.

🩺 Key Responsibilities

• Coordinate care and services for patients who meet Care Management criteria or require additional support.
• Conduct comprehensive assessments of patients' medical, functional, and social needs.
• Work directly with patients and families/caregivers to establish goals and appropriate interventions.
• Coordinate and support discharge planning.
• Assess patients' prior level of functioning, available support systems, and access to community resources.
• Help develop individualized plans of care with the multidisciplinary healthcare team.
• Assist with identifying appropriate care settings and continued service needs.
• Connect patients and families with community resources and support services.
• Collaborate with physicians and other members of the healthcare team on behalf of patients.
• Participate in rounds, case conferences, and multidisciplinary meetings.
• Advocate for patient needs and rights.
• Support appropriate utilization of healthcare resources and help identify strategies to reduce unnecessary length of stay.
• Monitor outcomes through data collection and variance tracking.
• Provide education regarding case management and discharge planning processes.
• Participate in multidisciplinary groups and development of guidelines.

✅ Required Qualifications

• Active LCSW license in Montana
• Minimum 1 year of clinical experience
• Graduate of an accredited school of Social Work, Sociology, or related field
• Hospital Care Management experience required
• Strong assessment, communication, care coordination, and discharge planning skills
• Ability to collaborate effectively with physicians and multidisciplinary healthcare teams
• BLS/CPR required

⭐ Preferred Qualifications

• 2 years of hospital-based Case Management experience
• ACM or CCM certification is preferred but not required
• Previous experience with hospital discharge planning and care coordination
