About the Position

Halcyon Nurse Staffing is seeking a qualified Physical Therapist (PT) to support patients at Encompass Health St. John Rehabilitation Hospital – Broken Arrow.

This opportunity is ideal for a licensed Physical Therapist interested in working in an inpatient rehabilitation hospital and providing therapy to patients as they work toward improved mobility, independence, and functional outcomes.

Requirements & Qualifications

Candidates must have:

• Current Oklahoma Physical Therapist license or applicable compact/recognized authorization to practice in Oklahoma
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification, as required by facility
• Experience providing physical therapy services in a healthcare setting
• Ability to evaluate patients and develop individualized treatment plans
• Strong knowledge of therapeutic exercises, mobility training, transfers, and functional rehabilitation
• Ability to collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the rehabilitation team
• Strong communication, documentation, and organizational skills
• Ability to work independently while following facility policies and clinical standards
• Must meet all facility and staffing credentialing requirements

Preferred Experience

Experience in one or more of the following settings is preferred:

• Inpatient Rehabilitation Facility (IRF)
• Acute Care Hospital
• Rehabilitation Hospital
• Skilled Nursing Facility (SNF)
• Post-acute rehabilitation

Responsibilities

The Physical Therapist will:

• Evaluate patients' physical functioning and rehabilitation needs
• Develop individualized treatment plans and establish measurable goals
• Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence
• Assist patients with transfers, gait training, and mobility activities
• Monitor patient progress and modify treatment plans as appropriate
• Document evaluations, treatments, progress, and patient outcomes accurately
• Educate patients and caregivers regarding exercises, mobility, safety, and home programs
• Collaborate with the interdisciplinary rehabilitation team
• Follow infection-control, safety, and facility policies

Credentialing Requirement

Candidates must be fully credentialed by October 12, 2026.
