Opportunity
Providence PACE Oregon is seeking an experienced NP or PA with geriatric experience to provide ongoing outpatient coverage. The practice serves adults age 55+ through a Program of All-Inclusive Care for the Elderly (PACE).
The role includes a combination of clinic-based care and community visits to patients in assisted living facilities, skilled nursing facilities, and private residences. Community visits are typically conducted alongside a nurse from the PACE team.
Schedule
- Monday–Friday, 8:00 AM–5:00 PM
- 3–4 days/week may be considered
- Minimum commitment: 2 weeks per month
- Full-time availability preferred
- 8 guaranteed hours/day
- Overtime applies after 9 hours
- No call
- Clinic and community-based visits
Patient Population
- Adults 55+
- Primarily geriatric patients
- Approximately 4–6 patients per shift
- No admissions required
- No hospital or surgical privileges required
Clinical Environment
You will work as part of an 11-member interdisciplinary PACE team, collaborating with nurses and other healthcare professionals to coordinate patient care. Most patient management and coordination is handled through the Epic EMR.
Requirements
- Active Oregon license
- DEA required
- Board Certified or truly Board Eligible
- BLS required
- Strong geriatric experience preferred/expected
- Current CV required for submission
Additional Details
- Support staff includes RNs, MAs/NPs/PAs, and office staff
- Provider will not supervise other APPs
- No resident or teaching supervision
- Malpractice insurance provided by the vendor
- No hospital privileges required
- Community visits may require driving to patient residences, assisted living facilities, or SNFs
Ideal Candidate
An NP or PA with strong geriatric/elder care experience who is comfortable providing both outpatient and community-based care and working collaboratively within an interdisciplinary PACE team.
