We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our long-term care team in Michigan.

The LPN will provide high-quality nursing care to residents in a skilled nursing/long-term care environment. This position works collaboratively with residents, families, physicians, registered nurses, certified nursing assistants, and other members of the interdisciplinary care team to promote resident safety, comfort, dignity, and quality of life.

The ideal candidate is a caring and professional nurse who enjoys building meaningful relationships with residents and is committed to providing safe, person-centered care.

Responsibilities

• Provide nursing care to residents in accordance with facility policies, physician orders, and applicable state and federal requirements.
• Perform resident assessments and monitor changes in condition.
• Administer medications and treatments accurately and safely.
• Monitor vital signs, symptoms, and resident responses to treatment.
• Document nursing care, assessments, medications, treatments, and changes in resident condition accurately and timely.
• Communicate significant changes in resident condition to the appropriate provider, RN, supervisor, and/or resident representative.
• Participate in development, implementation, and evaluation of individualized resident care plans.
• Assist with wound care, infection prevention, catheter care, respiratory treatments, and other nursing procedures within the LPN's scope of practice.
• Coordinate care with CNAs and other members of the healthcare team.
• Promote resident dignity, privacy, independence, and choice.
• Respond appropriately to emergencies and changes in resident condition.
• Follow infection-control, medication-safety, fall-prevention, and other facility protocols.
• Provide education and support to residents and families.
• Maintain accurate and confidential resident records.
• Participate in shift report and interdisciplinary communication.
• Support compliance with applicable Michigan and federal long-term care regulations.
• Perform other duties within the scope of the position as assigned.
