Location: Virginia Beach, VA 23464
Pay Rate: Competitive (Based on Experience)
Patient Care Setting: Home Health
Weekly Hour Guarantee: 40 Hours
Schedule: Monday-Friday, 8:30 AM - 5:00 PM
Productivity Expectation: 30 Points
On-Call Requirement: No
Weekend Requirement: No
Coverage Areas: Norfolk, Virginia Beach, Portsmouth, and surrounding communities
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Scrubs (Any Color)

Position Overview

We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team serving patients throughout Norfolk, Virginia Beach, Portsmouth, and surrounding areas. The Physical Therapist will provide individualized rehabilitative care in the home setting, helping patients improve mobility, restore functional independence, and achieve optimal outcomes while collaborating closely with an interdisciplinary healthcare team.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders and patient needs.
• Complete home health admissions, recertifications, discharges, routine therapy visits, and reassessments in accordance with agency and regulatory requirements.
• Deliver evidence-based physical therapy interventions focused on improving strength, balance, mobility, endurance, safety, and overall functional performance.
• Monitor patient progress, evaluate outcomes, and modify treatment plans as appropriate to achieve established goals.
• Educate patients, families, and caregivers on therapeutic exercises, fall prevention, home safety, and disease-specific rehabilitation strategies.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to ensure coordinated patient care.
• Maintain accurate, timely, and compliant clinical documentation.
• Meet established productivity expectations while delivering exceptional patient-centered care.

Required Qualifications:

• Active Physical Therapist (PT) License.
• Graduate of an accredited Physical Therapy program.
• Previous Home Health experience required.
• Strong assessment, clinical decision-making, and documentation skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while managing a field-based caseload.
