Description
Pride Health is actively hiring a Physical Therapist to join our client's Home Health Team.
Location: Fauquier County, Culpeper County, parts of Rappahannock County, and parts of Prince William County, Virginia
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Weekly Hours Guaranteed: 36 Hours
Productivity Expectation: 30 Points Weekly
On-Call/Weekend Requirement: None
Mileage Reimbursement: Yes, between patient visits (first patient to last patient)

Position Overview
We are seeking an experienced and compassionate Home Health Physical Therapist (PT) to provide high-quality rehabilitative care to patients in their homes throughout Fauquier, Culpeper, Rappahannock, and Prince William Counties, Virginia. The Physical Therapist will be responsible for evaluating patient needs, developing individualized treatment plans, and helping patients achieve optimal mobility, function, and independence.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders and patient goals.
- Provide skilled physical therapy interventions to patients in their homes, focusing on restoring function, improving mobility, reducing pain, and preventing disability.
- Complete Start of Care (SOC), recertification, reassessment, and discharge visits as required.
- Accurately document patient assessments, treatment plans, progress notes, and outcomes utilizing HomeCare HomeBase (HCHB) and PointCare systems.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other interdisciplinary team members to ensure coordinated patient care.
- Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and disease management strategies.
- Monitor patient progress and modify treatment plans as needed to achieve desired outcomes.
- Maintain compliance with federal, state, and agency regulations, policies, and quality standards.
Required Qualifications
- Current and unrestricted Virginia Physical Therapist (PT) License.
- Minimum of one year of recent Home Health Physical Therapy experience.
- Experience using HomeCare HomeBase (HCHB) and PointCare for clinical documentation.
- Strong assessment, clinical decision-making, and patient care skills.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Ability to travel throughout assigned coverage areas.
Preferred Qualifications
- Experience managing a diverse home health patient population.
- Knowledge of Medicare, Medicaid, and home health regulatory requirements.
- Excellent communication, organizational, and time management skills.
Work Environment
- Field-based position providing care in patients' homes.
- Travel throughout assigned Virginia coverage areas.
- Independent work environment with support from an interdisciplinary clinical team.
