Assignment: 16-week assignment; 6-month contract highly preferred
Schedule: Monday–Friday | 8 hours/day | 40 hours/week
Patient Scheduling: 60-minute evaluations; 30-minute follow-up visits
 

Job Summary:
 

The Physical Therapist will evaluate patients, develop and implement individualized treatment plans, and modify care based on patient needs, goals, physician direction, and clinical progress. The PT will provide quality outpatient therapy services focused on functional recovery and return to community activities.
 

Key Responsibilities:

• Perform comprehensive physical therapy evaluations for assigned outpatient patients.
• Develop individualized treatment plans and measurable goals based on clinical findings.
• Implement and modify treatment plans according to patient response and progress.
• Provide therapy services designed to promote functional recovery and return to community activities.
• Ensure treatment is provided under valid physician orders and within applicable insurance authorization.
• Complete accurate and timely medical record documentation, including:
  • Evaluations and treatment notes
  • Goal writing
  • Functional quality indicator assessments
  • Letters of medical necessity
  • Charge entry
• Provide patient and family education regarding treatment, progress, and care.
• Participate in student clinical education and provide appropriate supervision when assigned.
• Maintain professional development through continuing education, self-study, and professional activities.
• Follow applicable State Practice Act, regulatory requirements, and professional standards.
• Maintain professional licensure in good standing and complete required continuing education.
• Participate in quality/performance improvement initiatives.
• Maintain a safe environment for patients and staff, including safe patient handling, infection prevention, and workplace safety.

Required Qualifications:

• Degree in Physical Therapy from an accredited school
• Active Physical Therapist license applicable to the state where services are performed
• 6 months–1+ year of outpatient clinical experience
• CPR Certification
• Strong clinical reasoning, documentation, patient-care, and communication skills

Preferred Qualifications:

• 6-month contract commitment highly preferred
• Previous successful assignment with the staffing agency – highly desired
• Previous experience with Select Medical – nice to have

Work Environment / Expectations:

• Outpatient physical therapy setting
• Monday–Friday schedule
• 8-hour shifts / 40 hours per week
• 60-minute evaluations and 30-minute follow-ups
• Must maintain confidentiality and comply with organizational policies and Code of Conduct.
• Must demonstrate commitment to patient and employee safety.
• Must maintain professional appearance, attendance, and punctuality.
• Must demonstrate behaviors consistent with Select Medical's patient-care and organizational standards.
