Schedule: Monday–Friday | 8 hours/day | 40 hours/week
Patient Scheduling: 60-minute evaluations + 30-minute follow-up visits

Important: Therapist must be willing to float to Aurora Quincy as needed.
 

Job Summary:
 

The Physical Therapist will evaluate assigned outpatient patients, develop and implement individualized treatment plans, and modify care based on patient needs, goals, physician direction, and clinical progress. The PT will provide functional treatment programs designed to promote recovery and return patients to community activities.
 

Key Responsibilities:

• Perform comprehensive physical therapy evaluations for assigned patients.
• Use clinical reasoning to develop appropriate treatment plans and measurable goals.
• Implement and modify treatment plans based on patient response and progress.
• Provide individualized outpatient therapy focused on functional recovery.
• Ensure care is delivered under valid physician orders and within insurance authorization.
• Complete accurate and timely medical record documentation, including:
  • Evaluations and treatment notes
  • Goal writing
  • Functional quality indicator assessments
  • Letters of medical necessity
  • Charge entry
• Educate patients, families, students, and coworkers regarding therapy and patient care.
• Delegate and direct discipline-specific therapy services as appropriate.
• Participate in student clinical education and provide appropriate supervision when assigned.
• Maintain professional development through continuing education, self-study, and professional activities.
• Follow applicable State Practice Act, regulatory requirements, and professional standards.
• Maintain professional licensure in good standing.
• Participate in quality and performance improvement initiatives.
• Maintain patient and employee safety, including safe patient handling, infection prevention, and workplace safety.
• Float between assigned outpatient locations, including Aurora Quincy, as required.

Required Qualifications:

• Degree in Physical Therapy from an accredited school
• 6 months–1+ year of outpatient clinical experience
• Active/applicable PT license for the state where services will be performed
• CPR Certification
• Strong clinical reasoning, documentation, patient-care, and communication skills
• Ability and willingness to float to Aurora Quincy

Schedule & Patient Volume:

• Monday–Friday
• 8-hour shifts
• 40 hours/week
• 60-minute evaluations
• 30-minute follow-up visits
• Outpatient physical therapy setting
