Description
Pride Health is hiring a Physical Therapist to support our client's healthcare facility in Madison, OH
Job Details:
Schedule: Monday–Friday | 8:00 AM–4:30 PM
Location: Madison, OH
Float / Coverage Area: Ashtabula, Lake, and surrounding counties
Job Type: Local contract
Contract Length: 13 Weeks
Mileage Reimbursement Rate (if applicable): $0.49/ mile
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Perform comprehensive physical therapy evaluations in the home setting.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Provide skilled PT interventions to improve mobility, strength, balance, coordination, and functional independence.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Educate patients and caregivers on exercises, safety precautions, mobility techniques, and home programs.
- Coordinate patient care with physicians, nurses, therapists, and other members of the interdisciplinary team.
- Complete accurate and timely clinical documentation in accordance with home health and regulatory requirements.
- Manage an assigned caseload while meeting productivity expectations.
- Communicate changes in patient condition and progress to the appropriate care team.
- Maintain compliance with applicable clinical, safety, and documentation standards.
Requirements:
- Education: Doctor of Physical Therapy (DPT) or equivalent accredited PT program
- License(s): Active Physical Therapist (PT) License – Ohio
- Certification(s): BLS/CPR preferred
- Years of experience: Home Health PT experience required
- Skills/Specialties/Technology: Home Health, PT Case Management, Patient Evaluations, Treatment Planning, Mobility & Functional Training, Care Coordination, Clinical Documentation, HCHB/Homecare Homebase preferred, EMR, OASIS familiarity preferred
