Physical Therapist (Home Health) - Mountlake Terrace, WA

Amergis Healthcare Staffing is seeking experienced Physical Therapists for a 13-week Home Health contract serving patients throughout South King County, WA. This opportunity offers a Monday through Friday schedule, mileage reimbursement between patient visits, and the chance to make a meaningful impact by helping patients improve mobility, function, and overall quality of life in their homes.

• Schedule: Monday-Friday, 8:00 AM-4:30 PM
• Weekly Hours: 40 (36-hour guarantee)
• Coverage Area: South King County
• Charting System: HCHB
• Interview: Virtual or In-Person

Compensation

• Local Pay: Up to $2,320/week ($58/hour)
• Travel Pay: Up to $2,593/week
  • Taxable Rate: $30/hour
  • Meals Stipend: $602/week
  • Housing Stipend: $791/week

Responsibilities

• Complete comprehensive physical therapy evaluations and develop individualized plans of care.
• Assess patient functional status and collaborate with physicians and interdisciplinary team members.
• Provide skilled therapy services to restore function, improve mobility, reduce pain, and maximize independence.
• Monitor patient progress and communicate changes in condition to the care team.
• Educate patients, families, and caregivers regarding treatment plans, goals, and home exercise programs.
• Complete timely and accurate documentation in HCHB in accordance with agency and CMS guidelines.
• Supervise PTAs and Home Health Aides as permitted by state regulations.
• Participate in case conferences, in-service education, and quality improvement activities.
• Travel throughout assigned service areas to provide patient care.
• Prepare discharge summaries and progress reports as required.

Requirements

• Active Washington Physical Therapist License
• Minimum 6 months to 1 year of Physical Therapist experience
• Previous Home Health experience required
• Experience with HCHB EMR preferred
• OASIS experience required
• Strong clinical assessment and documentation skills
• Reliable transportation for patient visits
