Description
Registered Nurse (RN) – Home Health
Location: Westminster, MD 21157
Regular Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Coverage Area: Howard and Carroll Counties, MD
Position Overview:
We are seeking an experienced and compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will be responsible for conducting patient assessments, coordinating individualized care plans, performing specialized nursing procedures, and supporting patients with complex medical needs.
This position is ideal for a Home Health RN with strong clinical skills, experience in wound care and advanced nursing procedures, and a commitment to delivering high-quality, patient-centered care.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and established care plans.
- Conduct comprehensive patient assessments, monitor clinical conditions, and communicate changes in patient status to physicians and the interdisciplinary care team.
- Perform wound care, wound vac management, central line care, Foley catheter care, and other skilled nursing procedures.
- Provide PleurX drain management, venipuncture, and Foley and suprapubic catheter insertion, as appropriate to clinical competency and orders.
- Educate patients and caregivers on medication management, disease processes, treatment plans, and home safety.
- Maintain accurate and timely clinical documentation and coordinate care to support positive patient outcomes.
Schedule & On-Call Requirements
- Guaranteed hours: 32 hours per week.
- Regular schedule: Monday–Friday, 8:00 AM–4:30 PM.
- On-call requirement: One weekday night per week, from 4:30 PM to 8:00 AM the following morning.
- Weekend/on-call coverage: On-call participation is required; additional weekend rotation details to be confirmed.
- Mileage reimbursement: Yes, mileage between patient visits is reimbursed, including travel between the first and last patient visits.
Required Qualifications
- Active Registered Nurse (RN) license.
- Previous Home Health nursing experience.
- Strong clinical assessment, patient care, and documentation skills.
- Ability to provide independent in-home nursing care and participate in the on-call rotation.
